garbage, despite the facility having a policy and procedure titled “Garbage Disposal” that required that the garbage dumpster lids must be closed shut every time after disposal. In addition, it was noted that the facility did not ensure infection control protocols were followed. Specifically, while providing wound care to a resident, a Licensed Practical Nurse was observed on five different occasions either not washing her hands or performing hand hygiene. Also, it was observed that residents were not always provided with hand wipes or taken to wash hands prior to eating meals.
